Once determined, this information is then useful in identifying suitable corrective actions or modifications that may be helpful to prevent similar repeat failures with the associated benefits of improved reliability, reduced down time and improved safety.

The presentation will give a summary of 3 case studies relating specifically to corrosion and stress corrosion cracking failures that had recently occurred in WA industry. The steps involved in each investigation will be detailed with the mode and cause of the failure identified in each case with suitable recommendations for reducing the occurrence of similar failures given.
